1.) Understand the Purpose of a Care Plan

Being specific and detailed in care plans is crucial – this ensures that expectations are understood and met, and the care remains consistent. This includes providing a clear mandate to the support team, determining the person or individuals responsible for meeting specific objectives clearly and explicitly defining how data will be collected to monitor and report progress.

5.) Incorporate Technology

Technological advancements have made it easier to create and share care plans. Digital care plans make it accessible for all members of the support team, keeping the care plan accurate and up to date. Electronic care plans can also produce measurable results, providing the feedback needed to modify care plans continually.
